You have to follow a 2 week pre op diet in order to make your liver smaller before surgery.

You do this by following a low carbohydrate, very low calorie, high protein diet for 2 weeks before your surgical procedure.

There are no standardized guidelines for the pre weight loss surgery diet. That means the form the low carbohydrate, very low calorie, high protein diet takes is debatable amongst surgery practices.

Some programs allow only liquid shakes to achieve this, and some allow shakes + food. There is not ONLY one right way! You have options.

Pre Op Bariatric Macros

What’s most important is achieving the following preop bariatric macros as they define what a low carbohydrate, very low calorie, high protein diet really is:

  • 800-1200 calories per day
  • Less than 50 grams carbohydrate per day

60-100 grams protein per day

Bariatric Liquid Diet Schedule

Here’s a sample schedule of the 2 week liquid diet plan. Drink the first shake within an hour of waking up, then one every 3 hours throughout the day:

  • 7 am: Meal Replacement Shake 
  • 10am:Meal Replacement Shake
  • 1 pm: Meal Replacement Shake
  • 4 pm: Meal Replacement Shake
  • 7 pm: Meal Replacement Shake

Using liquid meal replacement shakes eliminates any confusion about what and how much to eat.

This assures you get the correct calories and bariatric macros without having to weigh and measure or calculate anything yourself.  It also gives you a break from preparing food. So that’s kinda nice!

Note:

A "meal replacement shake" is NOT the same thing as an over the counter "protein shake," "protein drink," or "protein supplement." 

Meal replacement shakes like this one have small amounts of added carb, fat, and fiber, like a real whole food meal would. This helps curb hunger and makes the liquid diet easier to follow.

HOW to Prepare the Meal Replacement Shakes

The bariatric specific high protein meal replacement is a powder you simply mix with water:

  • Use as much or as little water as you want
  • Ice or no ice. It’s up to you. Many people find “the colder the better.”

Use a shaker cup or blender. It’s up to you.

2 Week Pre Op Diet:
HOW Do I Vary the Taste of Meal Replacement Shakes?

Inevitably you’ll get sick of the shakes.

If you drink 5 meal replacement shakes a day for one week, that’s 35 shakes!

  • (5 shakes x 7 days = 35)

If you’re doing a 2-week pre op liquid diet then you’ll be drinking 70 shakes over 2 weeks!

  • (5 shakes x 14 days = 70)

Sooooo….

Vary Flavor

Use any no calorie or low calorie beverage in place of water. Try sugar free lemonade or tea or decaf coffee for example.

Use herbs as flavor enhancers:

  • Mint leaves
  • Ginger (ground or fresh)
  • Cilantro
  • Parsley

Use spices as flavor enhances:

  • Cinnamon
  • Pumpkin pie spice
  • Apple pie spice

Use baking extracts as flavor enhancers:

Almond, coconut, banana, hazelnut, maple, anise, lime, orange, pumpkin pie spice, peppermint, strawberry, or even cake batter extract!

Vanilla Meal Replacement Drinks

Add the following flavorings to vary your vanilla drink.

Note: Start with a small amount of extract if you’re not sure how much to use. You can always add more if it’s not strong enough, but you can’t take it back if you add TOO much!

  • Piña Colada: Add ¼ to ½ teaspoon coconut and ¼ to ½ teaspoon pineapple extract
  • Eggnog: Add ¼ to ½ teaspoon rum extract. Sprinkle with nutmeg
  • Nutty Almond: Add ¼ teaspoon almond extract
  • Creamsicle: Use FLAT diet orange soda or orange-flavored Crystal Light® in place of water
  • Latte: Use decaffeinated coffee in place of water

Chocolate Meal Replacement Drinks

Add the following extracts to vary the flavor of your chocolate drink. Remember: Start with a small amount of extract if you’re not sure how much you’ll like. You can always add more if it’s not strong enough, but you can’t take it back if you add TOO much!

  • Chocolate Almond: Add ¼ to ½ teaspoon almond extract
  • Chocolate Mint: Add ¼ to ½ teaspoon mint or peppermint extract
  • Chocolate Orange: Add ¼ to ½ teaspoon orange extract
  • Chocolate Banana: Add ¼ to ½ teaspoon banana extract
  • Mocha: Use decaffeinated coffee in place of water 

A Special Note for Diabetic Patients:
HOW to Modify the 2 Week Pre Op Diet

If you use insulin and/or oral medication(s) to treat your diabetes, you’re at risk of experiencing low blood sugars as you reduce carbs and calories on the bariatric 2 week liquid pre op diet.

Contact your doctor to discuss your blood sugar management before starting the pre surgery liver shrinking diet plan. It’s likely you’ll need to reduce your insulin or oral medication because the bariatric preop diet is low in carbohydrate and calories and will make your blood sugars go lower than normal. 

Monitor your blood sugar carefully and contact your doctor as needed
