Good news: The 1- 2 week pre op diet can be done with meal replacements or real food.

Surgery practices vary in what they recommend because there are no standardized guidelines for the pre weight loss surgery diet. Some programs allow only liquid shakes, and some allow shakes + food. Honestly either way works. There is no true need to be on liquids only.

Achieving the macronutrient composition:

  • Less than 1000 calories per day
  • Less than 50 grams carbohydrate per day
  • 60-100 grams protein per day

is really what matters on the 2 week pre op diet.

Example #1: 
2 Week Pre Op Diet with Meal Replacements 

Using liquid meal replacement shakes eliminates any confusion about what and how much to eat. This assures you get the correct calories and bariatric macros without having to weigh and measure or calculate anything yourself.  It also gives you a break from preparing food. So that’s kinda nice!

You need to know that a "meal replacement shake" is NOT the same thing as an over the counter "protein shake," "protein drink," or "protein supplement."  

Meal replacements have small amounts of added carb, fat, and fiber, (like a real meal would), which helps curb hunger and makes the liquid diet easier to follow.

The high protein meal replacement powder is simply mixed with water and consumed 5 times per day. A 2 week bariatric liquid diet schedule would look something like this if you drink a shake every 3 hours throughout the day:

  • 6am
  • 9am 
  • 12pm
  • 3pm
  • 6pm

+ Drink 64 ounces of low calorie, sugar free, caffeine free beverages throughout the day.

+ “Free foods” like sugar free jello, sugar free popsicles, and broth or miso are allowed.

This is an example of a bariatric specific high protein meal replacement.

Modified Option

If you want to have a little something to chew on, you can follow the 1- 2 week pre op diet in the following way too. My bariatric program allows this as an option:

  • 4 bariatric specific meal replacement shakes
  • 1 meal consisting of vegetables only + a tiny bit of fat

A menu would look something like this:

  • Breakfast: Meal Replacement Shake
  • Snack: Meal Replacement Shake
  • Lunch: Meal Replacement Shake
  • Snack: Meal Replacement Shake
  • Dinner: 2 cups of low carbohydrate vegetables + 1 teaspoon of oil

+ Drink 64 ounces of low calorie, sugar free, caffeine free beverages throughout the day

+ “Free foods” like sugar free jello, sugar free popsicles, and broth or miso are allowed

Example #2:
2 Week Pre Weight Loss Surgery Diet with Protein Shakes + Food

You can accomplish the same bariatric liver shrinking diet by using protein shakes + food. In this case, it's ok to use over an over the counter protein shake, protein drink, or powdered protein supplement because you're eating a mix of real food too...so your shakes don't need to be full out meal replacements. Your daily meal plan would look like this:

  • 2-3 protein shakes 
  • 1 meal with less than 20 grams carbohydrates
  • Low carb snacks

A menu would look something like this:

  • Breakfast: Protein Shake
  • Snack: ½ cup low fat cottage cheese
  • Lunch: Protein Shake
  • Snack: 1 cup cherry tomatoes/cucumber slices
  • Dinner: 3-4 ounces Lean Protein + 2 cups Vegetables + 1 teaspoon of oil

+ 64 ounces of low calorie, sugar free, caffeine free beverages throughout the day

This route isn't as much of a no-brainer as using the bariatric meal replacement shakes. You have to make some decisions about what and how much to eat if you go this way, but at least you get to chew!

Keep a food log while you’re following the 1- 2 week pre op diet to keep track of calories, carbs, and protein.
